- One-time Application Processing Fee of BDT 500/- for Lyricist, Composer, and Legal Heir in favour of BLCPS Bank Account (payable via bKash, Rocket, Nagad, Upay, Cash, Cheque, or Pay Order)
- Identity document: Bangladeshi nationals may apply with NID or Passport. Foreign nationals must upload Passport copy.
- TIN Certificate: upload is required.
- Address Proof if Communication Address differs from Permanent Address.
- Passport / Driving License copy is optional for Bangladeshi nationals.
- 1 (one) passport size colour photograph (soft copy).
- Work Notification Forms for each Musical Work (minimum ONE released/published):
• Film Music / Non-Film: Digital release links with credits
• Background Music: Public release links, official credits, or production confirmation links
• Advertising: Campaign/video links and proof of authorship - Payment routing: Foreign nationals are paid by wire transfer only (bank details required). Bangladeshi nationals may use approved local payment methods.

By filling out and submitting this membership form, I hereby declare and confirm that:
1. I intend to become a member of BLCPS and confirm and warrant that I will abide by the rules, regulations, terms and conditions, and code of conduct laid down, from time to time, by BLCPS.
2. By expressing my intention to be a member of BLCPS and upon signing this Membership Form, I hereby authorize BLCPS to act on my behalf with regard to my Copyright and/or Neighboring Rights through: (i) Issuing licenses to the public or third parties, (ii) Collecting royalty, (iii) Monitoring rights to identify infringements and (iv) Taking appropriate legal action/s.
3. I hereby confirm that BLCPS shall retain 20% of the royalty collected from the issuance of licenses for my work and disburse the rest in my favor.
4. BLCPS may retain my information and may store, preserve, use, and share the same for its function or objectives, including marketing purposes.
5. I would like to receive BLCPS emails containing helpful information, including advice, industry events or invites, and news.
6. I have no objection to BLCPS sharing my contact details with 3rd parties so that these organizations can contact me directly about opportunities available and services that may be of interest.
7. Declarations: I hereby authorize BLCPS to represent me in the interest of protecting my copyrights and/or neighboring rights under the Copyright Act, 2023. I warrant and declare that all information furnished is true and correct and that my registration will be subject to cancellation if information is inaccurate or I violate any membership rules or conditions of BLCPS.
